Information about=20 League Fees, Uniform, and Additional Costs

The FTGS = League Fee=20 covers participation in the league, a game jersey shirt, one pair of = socks, and=20 a visor.

The League = Fee is $100=20 for the first child, $180 for two children, and $230 for three or more = children.=20 You

can also = purchase game=20 shorts ($12 gray/silver color) and / or extra socks ($5 pair).

Each girl = will need to=20 have her own softball glove/mitt prior to her first practice. Softball = cleats=20 are highly

recommended = but not=20 required. Bats, balls, helmets, and catcher=E2=80=99s equipment are = provided by the=20 league.

Each girl can = bring and=20 use her own ASA approved equipment or gear.

and approval = for my=20 child to participate as a member of a softball team which is a part of = the=20 Franklin Township Girls'

Softball = League.

I understand = that there=20 are certain risks of damages and injuries, including death, inherent in = the=20 practice and play of

softball, as = well as in=20 traveling and in other related activities incidental to my child's=20 participation, and I am willing to

assume these = risks on=20 behalf of my child. These risks include but are not limited to those = hazards=20 associated with weather

conditions, = travel,=20 playing conditions, equipment and other participants.

I understand = that=20 sliding into base is dangerous to my child and other players and may = result in=20 serious injury or death.

I understand = that the=20 very nature of the game of softball is hazardous and risky, including, = but not=20 limited to, the acts of

pitching, = throwing,=20 fielding and catching of the ball, the swinging of the bat, running, = jumping,=20 stretching, sliding, diving,

and = collisions with=20 other players and with stationary objects, all of which can cause = serious injury=20 or death to my child

and to other = players.=20

Further, I = agree that=20 in consideration for the right to allow my child to participate as a = member of a=20 team in the Franklin

Township = Girls=E2=80=99=20 Softball League, and in consideration for permission to play on the = fields=20 arranged for by the team or

members of = the Franklin=20 Township Girls=E2=80=99 Softball League:

1. On behalf = of my=20 child and myself, I do voluntarily, elect to accept and solely assume = all risks=20 of injury incurred or

suffered by = child (a)=20 while practicing or playing as a member of the Franklin Township = Girls=E2=80=99 Softball=20 League, (b) while

serving in a=20 non-playing capacity as team member or observer during practice or play = by other=20 teams or by other players

on my child's = team, and=20 (c) while on or upon the premises of any and all of the fields arranged = for by=20 my team or Franklin

Township = Girls=E2=80=99=20 Softball League for practice or play.

2. In = addition to=20 giving my full consent for my child's participation, I do hereby waive, = release,=20 discharge and agree not to

sue the = Franklin=20 Township Girls=E2=80=99 Softball League, the owner or operator of any = field or other=20 entity designated, the

Amateur = Softball=20 Association of America, or their owners, officers, agents, servants,=20 associations, employees, or any

person or = entity=20 connected with the team, Franklin Township Girls=E2=80=99 Softball = League, field or=20 Amateur Softball Association

of America = for any=20 claim, damages, costs including attorneys fees, or cause of action which = I or my=20 child have or may

have in the = future as a=20 result of damages, injuries, including death, sustained or incurred by = my child=20 from whatever

cause = including but not=20 limited to the negligence, breach of contract or wrongful conduct of the = parties=20 hereby released.

I hereby = certify that=20 my child is fully capable of participating in the designated sport and = that my=20 child is healthy and has

no physical = or mental=20 disabilities or infirmities that would restrict full participation in = these=20 activities, except as made

known to = coaches and=20 officials of the team and Franklin Township Girls=E2=80=99 Softball = League.

I further = agree on=20 behalf of myself and my child or children listed below, that I shall = hold=20 harmless and fully indemnify the

parties = hereby released=20 from any and all claims, damages, costs including attorney fees, and = causes of=20 action which may

arise from = any cause of=20 action made by me or by, through or on behalf of my child, even if the = damages,=20 injuries or

death are = caused in=20 whole or in part by any of the persons or entities hereby released. =

I ACKNOWLEDGE = THAT I=20 HAVE READ AND THAT I UNDERSTAND EACH AND EVERY ONE OF THE ABOVE = PROVISIONS=20 IN
